Healthy Hearts is owned and managed by Thrive Tribe Ltd. We were selected as the chosen provider of a Cardiovascular Disease Prevention Service in Hammersmith & Fulham, Kensington & Chelsea and Westminster. This service is funded by the, London Borough of Hammersmith and Fulham, Royal Borough of Kensington and Chelsea and Westminster City Council and commissioned and managed by the Three Boroughs’ Public Health Service. We are looking forward to providing a high quality, evidence based community service that will help tackle London’s biggest killer.
The service will help over a thousand local people each year with intensive programmes focused on weight management, healthy eating, physical activity and other CVD risk factors. The service will be provided by a multi-disciplinary team including Dieticians, Nurses and weight management practitioners. We will be working in partnership with Better (GLL), the local leisure provider, as well as a range of local voluntary and community organisations.
When did the service commence?
The contract commenced on 1st October 2015 although it is likely to be January 2016 before we are providing the full programme in all three Boroughs. Please bear with us during the mobilisation phase while we recruit and train our team. People can be referred to the service from now and we will aim to contact new referrals from 1st October 2015 within 48hrs and we hope to be able to offer initial appointments by late November.
Who can access the service?
Residents of the three Boroughs who have been assessed at being at moderate to high risk i.e. at 10% or greater risk of CVD over the next 10 years can be referred into the service, or can self-refer. We also accept patients with diabetes, obesity and other health conditions which increase risk. Click here to send a secure email referral directly to The NHS team.

Who are Thrive Tribe?
Thrive Tribe is a specialist healthy lifestyle service provider. The organisation was set up by a small team of NHS public health staff in 2008 and has since become a respected provider of healthy lifestyle services in London, Suffolk and Luton.
