Cardiovascular disease or CVD as it’s sometimes described is an umbrella term for a number of heart and vascular conditions, the main four being coronary heart disease, strokes, peripheral arterial disease and aortic disease. It is cited as the UK’s biggest killer. Often CVD is a preventable disease and the onset can be caused by a lot of different lifestyle factors.
Your risk score is calculated on a number of factors including family history, your general health and wellbeing and your lifestyle choices. This risk then tells you the likelihood of you suffering with a form of cardiovascular disease over the next 10 years.
Some risk factors are defined by your age or ethnicity and there is nothing you can do about this, however many risk factors are dictated by lifestyle and behaviours and these are the ones we will focus on supporting you with.

The Healthy Hearts service provides clients at risk of Cardiovascular Disease with a one hour face-to-face assessment near to their home. During this session a number of measurements are taken, including cholesterol, BMI and blood pressure, along with assessment of risk factors such as a poor diet and low levels of physical activity.
Clients come away with a personalised Healthy Heart Plan which opens the door to a range of FREE programmes designed to reduce the risk of CVD, including physical activity classes, weight management groups, healthy cookery classes and specialist clinics. The programmes are provided by a range of health and wellbeing professionals including dieticians, nurses and physical activity practitioners.
The service started in October 2015 and welcomes self referrals and referrals from Health Professionals. We have a range of clinics, health clubs and activities running across Hammersmith & Fulham.
The Healthy Hearts team offer sessions at varying times and days of the week. Morning, afternoon and evening appointments or sessions are currently available Monday to Friday.
Healthy Hearts is part of Thrive Tribe, which is a specialist healthy lifestyle service provider. The organisation was set up by a small team of NHS public health staff in 2008 and has since become a respected provider of healthy lifestyle services in London, Suffolk, East Sussex, Norfolk, Oxfordshire and Luton.
